Children and Young People need to be referred by an agency such as Social Work, Education, Housing, Health Professional or Voluntary Sector. If you need assistance or would like to know more about how to make a referral please contact Amanda Brown.
Adults - 16 + Years
To join the scheme a adult will need to fill out an application form or an agency working with them can complete one on their behalf. if you need assistance please contact Ian Edwards
Adults - 60+ Years affected by Dementia
